The Office of Intellectual and Developmental Disability Supports (OIDD)is working with a group called Qlarant Quality Solutions to hear from families of children with disabilities about their services. This helps Louisiana learn what is working and what needs to improve. OIDD is asking for your help with an important survey:

Child Family Survey (CFS)

  • Mailed to families who have a child under age 18
  • The child must live at home and receive OIDD services
  • The survey comes with a prepaid envelope so you can mail it back easily or complete the survey online with the survey code
  • It is anonymous – no names are used
  • The survey lets you share how you feel about the services your child and family receive
  • You got a letter from Bernard Brown, OIDD Assistant Secretary with the survey

If you received this survey, please use the survey code to enter your survey responses directly online at https://odesa.nationalcoreindicators.org/begin/cfs2026. You can also fill it out and mail it back with the prepaid envelope.

The deadline for this survey is September 30, 2026.
