LaDDC News: Help Make Services Better in Louisiana

The Office for Citizens with Developmental Disabilities (OCDD) is working with a group called Qlarant Quality Solutions to hear from families and people with disabilities about their services. This helps Louisiana learn what is working and what needs to improve. OCDD is asking for your help with two important surveys:

Child Family Survey (CFS)

• Mailed to families who have a child under age 18
• The child must live at home and receive OCDD services
• The survey comes with a prepaid envelope so you can mail it back easily
• It is anonymous – no names are used
• The survey lets you share how you feel about the services your child and family receive
• You got a letter from Bernard Brown, OCDD Deputy Assistant Secretary with the survey

If you received this survey, please fill it out and mail it back with the prepaid envelope!

Adult In-Person Survey (IPS)

• For adults (18 and older) who receive OCDD services
• A person from Qlarant will call or email to set up a time to talk
• The survey can be done in-person or virtually
• You can have a caregiver or family member with you if you want
• You can stop the survey at any time
• Your answers are private
