The Office of Intellectual and Developmental Disability Supports (OIDD) sent a Support Coordination Satisfaction Survey to people who receive support coordination through their home and community-based services.
The survey was sent directly to people by email. OIDD used the email addresses listed in the LaSRS system. Because the survey was sent directly to each person, there is no public link to share.
If you received the survey, the Council encourages you to complete it as soon as you can. Your answers can help OIDD learn more about your experience with support coordination.
OIDD will send reminder emails to people who received the survey but have not completed it yet.
The survey closes September 6, 2026.
