The Louisiana Developmental Disabilities Council is asking for your help. If you took part in any Council-supported activities during FFY25 (October 1, 2024- September 30, 2025), we want to hear from you.

We must report information about our activities to the federal government each year. Your feedback helps us do that. It also helps us learn what worked well and what we can improve.

This short survey is anonymous, which means your name is not collected. You can skip any question you do not want to answer.
