Guidelines and Resources for SOP Respondents

Applicants for Council funding are advised to read the Contract Funding Manual below and submit a complete Application for Funding packet. There is no guarantee of funding to the submitter of a proposal. The following documents/forms are required to be included in the application for funding:

- - SOP Application (found below)
  - W-9 Tax Form (if applicable)
    - - W-9 Form Instructions
  - Budget Form (save to your computer)
    - - Budget Form Instructions
  - Board Resolution (sample)
  - Signature of Authority/Disclosure of Ownership
  - 2 Letters of Support
  - Project Summary
  - Project Work Plan
